Антиоксидантна терапія у хворих з тяжкою черепно-мозковою травмою

We have studied the effects of a combined antioxidant treatment with 106 patients with a severe head injury. On the basis of the data of the chemiluminescence analysis of blood plasma taken from a jagular vein and that of the blood plasma of healthy donors we have stated the following. After the brain injury the central neurotic system of the patient suffers antioxidant stress; an increased activity processes of free radical oxidation correlated with the symptoms of neurone membranes injury, which was reflected in a high activity of aminotrasferase in spinal fluid. The antioxidant treatment including the injection of б-tocoferol, ascorbic acid and dexametason has not contributed to the normalisation of the processes of free radical oxidation in the patients' bodies. The additional continuous infusions of emoxipin in a dose of 10 mg/ kg per 24 hours improved the results of the chemiluminescence analysis. The reduction of emoxipin dose to 5 mg/ kg per 24 hours showed the weakening of the effect of the antioxidant treatment. The implementation of emoxipin in a dose of 1 mg/ kg considerably worsened the process on normalisation of free radical activity. The increase of the emoxipin dose to 15 mg/ kg per 24 hours has not showed any advantages to the its usage in a dose of 10 mg/ kg. Continuous infusions of emoxipin in a dose of 10 mg/ kg per 24 hours contributed to the elimination of the cerebral blood flow injuries revealed by means of reographic method; these infusions have not had any negative influence on the central hemodynamics state. At the same time the rejection of the usage of б-tocoferol in the scheme of the treatment has weakened the effect of the antioxidant treatment. In this work we state the following. In the brain protection from the oxidant stress the main meaning have both quick-acting water-soluble antioxidants (emoxipin) and well-known liposoluble ones (б-tocoferol) which strengthen the anti-radical defence of the neuronal structures in the course of time.
